How do I make a claim?
This depends on what you're claiming for. For money back benefits, like optical, consultation and surgical appliance, you receive and pay for your healthcare treatment, service or goods first and then send us your completed claim form with your receipt, within 13 weeks of the date of each payment. For cash payout benefits like maternity and in-patient, where you don't pay for anything upfront, you'll need to send us your completed claim form with other required supporting information.
